PETALUMA CITY SCHOOLS INTERDISTRICT TRANSFER AGREEMENT
I, _____________________________________, and my parent/guardian, in order that I may attend school in the Petaluma City Schools District as a student in good standing, agree to the following:
1. Maintain an attendance rate of at least 90% with no unexcused absences and be on time. Any absence of three days or more must be verified by a doctor's note indicating the nature of the illness. I understand that chronic tardies, i.e. more than three tardies per semester or trimester, may mean revocation of this agreement.
2. Maintain a grade point average of 2.0 or higher in all classes in any school term with no failing grades or making annual grade level progress in grades K-6. (NOTE: This contract shall not be revoked if a lack of academic progress is a direct result of an identified disability that requires special education services. However, a student is expected to show progress toward Individualized Education Program (IEP) goals)
3. Demonstrate positive, productive behavior in class(es) and on school grounds, before, during and after school and has no suspendable behaviors. Multiple instances of conduct violation or a school suspension during this contract period may result in the revocation of the Interdistrict Attendance Agreement by the Petaluma City Schools District.
4. Additional requirements/conditions:
Provided that I abide by the above regulations of this contract, I will be afforded all privileges accorded to students in the Petaluma City Schools District for the _______ school year.
